To register for this facility, please complete the reply slip and adhere to the following terms and conditions:
- All fields of the reply slip must be duly completed. Incomplete reply slip will not be entertained. If unsure, please call
6513 0388 or email edc@asme.org.sg. Closing date for registration is 17 February 2010.
- This service is limited to a maximum of one hour per appointment, on a first come first served basis. Priority will be given to
first timers. Appointment time will be allocated to you upon receiving the reply slip.
- The reply slip received will be taken as an agreement that you will attend the allocated session. All written cancellation
must reach us 5 working days for the clinics.
